सर्वद्वाराणि संयम्य मनो हृदि निरुध्य च ।
मूर्ध्न्याधायात्मनः प्राणमास्थितो योगधारणाम् ॥
ओमित्येकाक्षरं ब्रह्म व्याहरन्मामनुस्मरन् ।
यः प्रयाति त्यजन्देहं स याति परमां गतिम् ॥

Transliteration (IAST)

sarva-dvārāṇi saṁyamya mano hṛdi nirudhya ca |
mūrdhny ādhāyātmanaḥ prāṇam āsthito yoga-dhāraṇām ||

माम् अनुस्मरन्mām anusmaranand remembering Me.
यः प्रयातिyaḥ prayātihe who departs.
त्यजन् देहम्tyajan dehamgiving up the body.
सः यातिsaḥ yātihe attains.
परमाम् गतिम्paramām gatimthe supreme goal.
Translation

“Having closed all the gates of the senses, fixing the mind in the heart, and raising the life air to the crown of the head, one becomes established in yogic concentration. (12) And whoever, uttering the sacred single syllable Om, which is Brahman, and remembering Me at the same time, gives up his body and departs, attains the supreme goal. (13)”
